Appointment

The first step towards a beautiful, healthy smile is to schedule an appointment. Please contact our office by phone to schedule an appointment.

Cancellations

Your appointment time is reserved especially for you. If you are unable to keep an appointment please notify us immediately. We appreciate at least a 48-hour notice so that your appointment may be allotted to another patient.

Patient Forms

If you are a new patient to our office, please visit our patient forms to access our patient forms. Thank you and please contact our office if you have any questions at all.

Payment Options

For your convenience, we accept cash, personal checks, debit cards, and most major credit cards. Payment is expected at the time services are performed. Our accounts manager will be happy to answer your financial questions. She can be reached by calling our office at (727) 789-8900. 

    

Dental Insurance

We accept most major insurances and are happy to file insurance forms with any insurance company. Please bring your insurance card or information with you to your appointment so we can better assist you.

Please understand, however, that you are ultimately responsible for payment of all services. Should your insurance not cover what is estimated, you will be responsible to pay any uncovered portion.
